Urgent Care

Addressing non-emergency conditions that require prompt attention but aren’t life-threatening.

Women’s Health Services

Gynecological exams, birth control, and counseling on reproductive health issues.

Men’s Health Services

Screening for prostate health, testosterone levels, and counseling on reproductive health issues.

Pediatric Care

Comprehensive health services for children, growth and development assessments, and illness treatment.

Mental Health Care

Screening for and managing mental health issues such as ADHD, depression, anxiety, and stress-related disorders.

Diagnostic Tests

Ordering and interpreting common laboratory tests (e.g., blood tests, urinalysis) and imaging studies (e.g., X-rays).

Care Coordination

Coordinating care and referring patients to specialists when specialized medical intervention is needed.

Lifestyle Counseling

Guidance on nutrition, exercise, smoking cessation, and weight management.

Women’s Health Services

Evaluating a patient's risk for developing certain health conditions and providing guidance on mitigating those risks.

Patient Education

Teaching patients about their health conditions and treatment options, including self-management skills.

Skin Care & Minor Procedures

Treatment of minor skin conditions, removal of moles or warts, and other minor surgical procedures.

Non-operative Orthopedic Care/Sports Medicine

Treatment of acute and chronic musculoskeletal conditions, joint injections, strength and training program assistance, athlete assessment, and major sport competition strategic planning.

Home Care Services

Coordination of care for patients who require services at home, such as home visits for chronic illness management.

Coordination of Care:

Managing and coordinating care among various healthcare providers to ensure comprehensive treatment plans.

Practice Membership

Cost:
$150 monthly or
$1500 a year

You can choose to pay $150 per month or receive a discounted rate ($1500) for annual payment (equivalent to two months free).

Family Rate

Cost:
$400 monthly for two adults and two dependents
$350 monthly for two adults and one dependent
$75 monthly for each additional dependent membership

New Member One Time Sign-Up Admin Fee

Cost: $100 per member

This fee covers the administrative costs of enrollment. Upon becoming a member at Lost Creek Medical, your doctor will undertake the following tasks:

Inputting your information into the electronic record, Elation. Establishing your access to the patient portal and HIPAA compliant texting and calling service, Spruce. *** membership fees are collected after signing practice agreement with Dr. Bubis.
